<p>“As we pass through our later thirties, the stratum skin layer becomes more adherent,” Truswell explains. “[Dead cells] pile up and the skin looks dull. These cells are within the pores,” he notes. As dead cells become visible, lines, brown spots and red patches begin to form as well.</p>
<p>According to Levin, additional signs of aging that we can expect to see in our thirties include wrinkles, volume loss, bags under the eyes, more prominent nasolabial folds (laugh lines), jowling and uneven pigmentation.</p>
<p>“I always say that life starts to show up on our face and skin in our thirties,” she notes.</p>
<p>Recommended treatments</p>
<p>Levin recommends neuromodulator injections (Botox, Dysport, Xeomin) every 3-4 months to soften forehead and frown lines.</p>
<p>For pronounced wrinkles and sun damage, Truswell recommends medium-depth chemical peels, photo ​therapy (IPL) and CO2 laser facials. Levin, too, is a proponent of laser and IPL treatments as well as broadband light (BBL by Sciton) to address uneven skin tone and treat broken capillaries, blotchy red patches, brown spots and yellow pigment.</p>
<p>“My favorite treatment is the combination of BBL with Halo by Sciton,” admits Levin. “Halo is the first hybrid fractional laser resurfacing device that combines both ablative and non-ablative resurfacing treatments with two different wavelengths in one laser,” she explains.</p>
<p>According to Levin, Halo can be tailored to a specific condition or patient need, and achieve the same results as more invasive ablative lasers but with less downtime and fewer treatments.</p>
<p>Signs of aging in our forties</p>
<p>“Passing through our forties, things begin to descend, the jaw line slackens and jowls sag downward,” notes Truswell.</p>
<p>Signs of aging at this stage may include a ‘turkey wattle’ in the neck, loosening and hanging of the upper lids, fat bags pushing forward in the lower lids, descending eyebrows and further signs of dull skin.</p>
<p>Levin notes that we also begin to lose even more subcutaneous fat in our forties. “I tell my patients all the time, ‘Baby fat is real!’ While there is variation in genetics, environmental exposure and lifestyle behaviors, the first place we tend to lose fat is in our mid-face, temples and cheeks in front of the ears.”</p>
<p>Recommended treatments</p>
<p>A turkey wattle can be corrected through a neck lift procedure. However, many patients wait until they are in their fifties to have this done.</p>
<p>Aging in the eyelids and eyebrows can be addressed through blepharoplasty (eyelid surgery) and brow lifts, respectively.</p>
<p>Loss of subcutaneous fat can be disguised using injectable fillers. “In order to restore fullness, fillers are really the best non-invasive option,” explains Levin. “We live in a time with many different dermal filler options and — in the hands of a skilled and trusted dermatologist or plastic surgeon — fillers allow us to create a natural and rejuvenated appearance without looking ‘done.’”</p>
<p>Signs of aging in our fifties</p>
<p>“As more decades pass us by, we lose volume in the muscles and soft tissues of the face,” says Truswell. “The bony skeleton also undergoes changes. While everyone ages at a different rate and outside factors do contribute to the process (sun exposure, tobacco use, illness), as we move through the decades of life, facial changes accelerate more and more.”</p>
<p>According to Levin, this is when skin laxity, loss of collagen and elastin and increasing resorption of bone really start to show in the face and neck. Women lose about one-third of their collagen in the first five years of menopause, causing the skin to become drier, looser and more susceptible to inflammation, sun damage and stressors, such as pollution and underlying health issues.</p>
<p>Recommended treatments</p>
<p>“To address skin laxity, ultrasound-based or radiofrequency machines can stimulate collagen and elastin formation by stimulating fibroblasts and tightening skin in areas such as the brow, neck, chest, knees and jawline,” says Levin.</p>
<p>Ultherapy, which utilizes ultrasound energy, has the added benefit of creating pockets of micro-focused heat energy as deep as the muscle for further tightening beyond the skin alone. In addition to these devices, Levin recommends neuromodulator and dermal filler injections in the jawline and neck to minimize jaw laxity.</p>
<p>“Botox and Dysport can soften the platysmal muscle, which becomes more noticeable with age and contributes to neck laxity,” she explains.</p>
<p>Depending on the severity of sun damage, either a more aggressive fractional laser resurfacing treatment such as Halo or a fully ablative erbium or CO2 laser resurfacing treatment can significantly improve crepey skin and deep-set wrinkles, particularly around the mouth and eyes.</p>
<p>Your fifties may also be the time to start considering plastic surgery procedures, if you are experiencing more pronounced aging.</p>
<p>“A​s the soft tissues of the face descend they can be repositioned with various surgical lifting procedures depending on the degree of descent,” says Truswell. “These may include one of the facelift procedures or neck lift, for example. Excess skin and fat bulges in the eyelids can be treated through blepharoplasty,” he suggests.</p>
<p>Beyond your fifties</p>
<p>The same signs of aging mentioned earlier become more pronounced as more time passes. Many of the same treatments our experts recommend for patients in their fifties are also effective for older patients.</p>
<p>However, everyone ages at a different pace, so it’s best to consult an expert to determine what treatments will work best for your specific skin needs.</p>

<p>You may be wondering, “How in the <em>world</em> is inserting hundreds of little needles into your face relaxing? And why would anyone want to do that?” It sounds crazy, but microneedling has a ton of benefits, including:</p>

<p>enhanced product absorption</p>
<p>For anyone who’s looking for a way to tackle these concerns at home, microneedling might be your answer. Here’s what you need to know about this miraculous process.</p>
<p>What’s microneedling?</p>
<p>Microneedling, often referred to as dermarolling or collagen induction therapy, is a cosmetic procedure in which thousands of tiny little needles are inserted into the surface of skin via a rolling or stamping device.</p>
<p>Dermarolling works by creating microscopic wounds which induce collagen and elastin production. If you didn’t know, collagen is the most abundant protein found in the human body and is responsible for holding together connective tissue like skin, muscles, tendons, cartilage, and bones.</p>
<p>This lovely protein is also what keeps us looking young and gorgeous. Unfortunately, it’s believed that collagen production slows down by about 1 percent per year after the age of 20, which translates to the big A word — aging.</p>
<p>Despite how terrifying dermarolling may seem, it’s actually considered a minimally invasive procedure with little to no downtime. However, the recovery process does depend largely on the length of the needles used. Obviously, the longer the needles, the deeper the wound — and that means the longer the recovery time.</p>
<p>What size derma roller is best?</p>
<p>This will depend largely on what you’re trying to accomplish. Since we’re all about simplicity, here’s a table summarizing what length should be used depending on what you’re trying to treat.</p>

<p>Note: Microneedling won’t help postinflammatory erythema (PIE), which is redness or pink blemishes. And be aware that derma rollers or microneedling instruments that are greater than 0.3 mm in length are not approved or cleared by the Food and Drug Administration.</p>
<p>How to use a derma roller</p>
<p>Follow these steps <em>precisely</em> to avoid any hazards and unwanted infections.</p>
<p>Step 1: Disinfect your roller</p>
<p>Disinfect your derma roller by letting it soak in  for approximately 5 to 10 minutes.</p>
<p>Step 2: Wash your face</p>
<p>Thoroughly cleanse your face using a gentle pH-balanced cleanser. If you’re using a derma roller with needles longer than 0.5 mm, you’ll also need to wipe down your face with 70 percent isopropyl alcohol before the rolling process.</p>
<p>Step 3: Apply numbing cream, if needed</p>
<p>Depending on your pain tolerance, you might need to apply an anesthetic cream. However, you’ll most certainly want some numbing cream for anything above 1.0 mm, since that needle length <em>will</em> draw blood via pinpoint bleeding.</p>
<p>If you use numbing cream, follow the instructions the manufacturer provides, and make sure to completely wipe it if off before you start rolling!  is a great option.</p>
<p>Step 4: Begin derma rolling</p>
<p>The technique is very important, so listen closely! Splitting up your face into sections makes the whole process easier. Here’s a visual of what that looks like:</p>
<p>Avoid rolling in the shaded area, which represents the orbital (eye sockets) area.</p>
<p>Roll in one direction 6 to 8 times, depending on your skin tolerance and sensitivity, and make sure to lift the roller after each pass. So, roll in one direction. Lift up. Repeat.</p>
<p>Lifting the derma roller after each pass prevents the dreaded “track marks” that make you look like a cat clawed your face.</p>
<p>After you roll in the same place 6 to 8 times, adjust the derma roller slightly, and repeat. Do this until you’ve covered the entire section of skin you’re treating.</p>
<p>After rolling in one direction, it’s time to go back over the area you just rolled and repeat the process in the perpendicular direction. For example, say you finished rolling across your forehead <em>vertically</em>, now would be the time to go back and repeat that entire process <em>horizontally</em>.</p>
<p>By the end of this entire procedure, you should have rolled over each area 12 to 16 times — 6 to 8 horizontally, 6 to 8 vertically.</p>
<p>Contrary to popular belief, we <em>do not</em> need to roll diagonally. Doing so creates an uneven pattern distribution with more stress on the center. If you decide to do this, please be careful and take extra precautionary measures.</p>

<p>Step 5: Wash your face with water</p>
<p>After you’re done microneedling, rinse your face with water only.</p>
<p>Step 6: Clean your derma roller</p>
<p>Clean your derma roller with dishwasher soap. Create a soapy water mix in a plastic container, then swish around the roller vigorously, making sure the roller doesn’t hit the sides. The reason we use detergents like dish soap directly after rolling is because alcohol doesn’t dissolve the proteins found in skin and blood.</p>
<p>Step 7: Disinfect your roller</p>
<p>Disinfect your derma roller again by letting it soak in the 70 percent isopropyl alcohol for 10 minutes. Put it back in its case, give it a kiss, and store it somewhere safe.</p>

<p>We know that they can give you much more dramatic results and work on more significant skin issues, but are they okay for home use?</p>
<p><strong>MICRO NEEDLING DEVICE TYPES QUICK REFERENCE</strong></p>
<p>When talking about skin needling either professionally or at home there are a few different types of micro needling devices. Each of the device types is a little different.</p>
<p>The one you choose depends on your preferences and comfort level with the process.</p>
<p>MANUAL ROLLER</p>
<p>This is the most common type of roller for home treatment although they are also used by cosmetic clinicians.</p>
<p>One of their biggest draws is that they are really easy to use, even for beginners. Prices start around $10 and go up from there, but you don’t need to shell out a lot on an expensive device to get the benefits of treatment, but a good vitamin C serum will go a very long way.</p>
<p>MANUAL STAMP OR PEN</p>
<p>Stamps or pens are less common since they’re slightly harder to use. It’s a bit more difficult to apply even pressure across the whole treatment area as compared to a roller. It’s also more difficult to cover larger treatment areas (like the thighs or stomach) as compared to a roller or an electric device. Still, some people prefer them to rollers.</p>
<p>ELECTRIC STAMP OR PEN</p>
<p>Electric derma pens (also known as micro needling pens) are probably what you think of when you think of a professional micro needling treatment.</p>
<p>They have a small disposable head called a bayonet where the needles are located in a plastic casing. When the device is turned on the bayonet rapidly moves the needles in and out of the casing. The needles penetrate the skin at a very controlled depth because of the plastic casing acting as a guide against the skin.</p>
<p>ELECTRIC DERMA PEN FEATURES</p>
<p>Electric derma pens have a lot of good features including:</p>
<p>Adjustable speed and depth of needles for customizable treatment from 0.25mm to improve product penetration all the way to to 2.0mm to promote collagen formation and fill in acne scars and wrinkles.</p>
<p>Very user friendly and much more control over results since all you have to do is make sure the plastic casing is in contact with the skin in order to get the proper level of penetration.</p>
<p>Replaceable heads so the device can be used by more than one person.</p>
<p>Even though derma pens have these great features a lot of people shy away from them for home treatment because they seem like they would be much more difficult to manage compared to  using a microneedling a roller at home. Apparently that isn’t the case.</p>
<p>MICRO NEEDLING RESULTS</p>
<p>Micro needling results can be really impressive in general but comparison electric pen versus roller results isn’t so easy.</p>
<p>Although diagrams like this one are common in the promotional material of a lot of electric needling pen manufacturers, these claims aren’t confirmed by any of the clinical studies on micro needling.</p>
<p>MICRO NEEDLING ELECTRIC PEN VS ROLLER</p>
<p>The before and after shots and results from electric treatments to seem much more dramatic than those of manual rolling, but the reason may not be the angle of the needles.</p>
<p>It could be that the needles cover and penetrate more evenly, which results in more consistent results. It could also be that the speed of the electric device just makes way more micro-punctures than a roller.</p>
<p>There is no definitive answer as of yet but it does seem that electric devices provide more dramatic results and also offer more versatility in treatment options.</p>
<p>HOW TO  USE A DERMA PEN LIKE DR. PEN</p>
<p>Using an electric derma pen at home isn’t very different from using a manual micro needling roller. The steps are the same:</p>
<p>Disinfect device (if applicable).</p>
<p>Apply numbing cream if using and wait 15-20 minutes. Anaesthetic is generally recommended with an electric device. Make sure to wipe off all residue as otherwise it can clog up the machine.</p>
<p>Thoroughly cleanse face.</p>
<p>Apply pretreatment serum, preferably hyaluronic acid.</p>
<p>Turn device on at the right depth for your desired treatment.</p>
<p>Run the device over skin in all directions (up and down, side to side, and diagonally) about 5 times in each direction. Extra treatment can be done to scars or deep wrinkles.</p>
<p>Following treatment apply vitamin C serum.</p>
<p>After treatment skin feels hot and may sting like a mild to moderate sunburn. If skin feels extra tight following treatment a hyaluronic acid sheet mask can be applied for 15 to 20 minutes (before vitamin C serum) which gives skin a nice moisture boost. Follow with vitamin C serum.</p>

Yet so few of us know exactly what it means and why it&#8217;s important. So we asked top dermatologists to give us the scoop on our skin&#8217;s acid mantle.</p>
<p><strong>What is the acid mantle layer of skin and why is it important?</strong></p>
<p>The acid mantle is used to describe the acidic nature of your skin&#8217;s surface. &#8220;It&#8217;s this very thin film on the surface of our skin that acts as a barricade to bacteria and other viral substances trying to penetrate,&#8221; says Sandra Lee, M.D., a board- certified dermatologist and founder of SLMD Skincare. This slight acidity not only protects against bacteria, but also preserves the skin&#8217;s microbiome, ensures its structural integrity, and helps maintain its vital biological processes. “It’s our skin’s first line of defense,” she explains.</p>
<p><strong>What is the acid mantle made of?</strong></p>
<p>Skin&#8217;s acid mantle composed of sebum, which is the natural oil produced by the sebaceous glands in skin and our body’s sweat. “The acid mantle’s composition is slightly acidic, due to the combination of the amino acids from sweat and the triglycerides, fatty acids, and wax esters contained in the sebum,” M.D., a dermatologist in New York City and founder of Fifth Avenue Aesthetics.</p>
<p><strong>What causes the acid mantle to be disrupted and how can you tell if it&#8217;s damaged?</strong></p>
<p>There are several factors that can disrupt the acid mantle and the natural pH of skin. This can lead to dryness, redness, irritation, and even a more aged appearance. The most common include:</p>
<p><strong>Cleansers</strong><strong> and soaps: </strong>Cleansing can increase the pH of skin, damaging its acid mantle, but it usually returns to its natural pH within a few hours. This “return rate” can be greatly influenced not only by the pH of a product but also the type of cleansing ingredients used. Syndet cleansing bars, like Dove Beauty Bar, quickly return skin to it&#8217;s natural pH after use because they are slightly acidic/neutral and contain milder cleansers than traditional bar soap. Other bar soaps typically have high pH, so skin can take a longer time to return to its natural state after use.</p>
<p><strong>Your age: </strong>Research has shown that age can affect the pH of skin. Throughout our life span, our skin’s pH changes from near neutral pH of newborn skin, to slightly acidic (lower pH) within your first year of life, and then gradually becomes more alkaline (higher pH) as we age. As pH increases with age, the acid mantle can become compromised.</p>
<p><strong>Skin disease: </strong>Inflammatory skin diseases such as atopic dermatitis (eczema), rosacea, and acne are associated with higher pH and a disrupted acid mantle. Maintaining a normal pH is important for treating these conditions.</p>
<p><strong>How do you fix or repair the acid mantle?</strong></p>
<p>&#8220;When the acid mantle is disrupted, it allows for irritants and bacteria to enter,&#8221; Dr. Hayag says, so it&#8217;s crucial to protect skin&#8217;s natural barrier. Here are the best ways to restore the acid mantle and prevent future damage:</p>
<p><strong>Be gentle with cleansing: </strong>&#8220;Switch to cleansers that are gentle and contain fatty acids, ceramides, and other barrier-repairing ingredients that cleanse skin without destroying the acid mantle,” Dr. Hayag suggests. For the body, swap your bar soap with a syndet bar, which has a lower pH and is gentler on skin.</p>

<li><strong>Home skin care.</strong> Using sunscreen can help limit the contrast between unscarred skin and a scar. Some medicated creams, such as those containing azelaic acid or hydroxyl acids, might help too.</li>
<li><strong>Soft tissue fillers.</strong> Injecting collagen, fat or other substances under the skin can plump the skin over indented scars. The goal is to make the scars less noticeable. Results are temporary, so repeat treatments are needed to retain the effect. This method has little risk of changes in skin color.</li>
<li><strong>Steroid injection.</strong> Injecting steroids into some types of raised scars can improve the appearance of your skin.</li>
<li><strong>Laser resurfacing.</strong> This approach is increasingly popular and is often used on scars that were once treated with dermabrasion. This technique has an increased risk of side effects for people with darker skin or a history of keloids.</li>
<li><strong>Other energy-based procedures.</strong> Pulsed light sources and radiofrequency devices help make scars less noticeable without damaging the outer layer of skin. Results are subtle, and you might need repeat treatments.</li>
<li>This procedure is usually reserved for more severe scarring. Your doctor removes the top layer of skin with a rapidly rotating brush or other device. <li><strong>Chemical peel.</strong> Your doctor applies a chemical solution to the scar tissue to remove the top layer of skin and minimize the appearance of deeper scars. <li><strong>Skin needling.</strong> Your doctor rolls a needle-studded device over the skin to stimulate collagen formation in the underlying tissue. It&#8217;s a safe, simple and possibly effective technique for acne scarring. It has minimal risk of discoloring the skin. Results are subtle, and you may need repeat treatments.</li>
<li>Using a minor procedure called punch excision, your doctor cuts out individual acne scars and repairs the wound with stitches or a skin graft. With a technique called subcision, your doctor inserts needles under the skin to loosen fibers below a scar.</li>
<li><strong>OnabotulinumtoxinA (Botox).</strong> Sometimes the skin around acne scars puckers. Injecting Botox relaxes the surrounding skin, which may improve the appearance of an acne scar. Results are temporary, so repeat treatments are needed to retain the effect.</li>
